	To open an On-line Account:
	Complete the Coltrane New Account Form We have instituted a Client Identification Program in our obligations under the USA Patriot Act. For more information, please review the Customer Identification Program Notice Complete the On-line Account Agreement & Enrollment Form. You will have free on-line access to your account with the ability to enter trades, view your account activity and receive quotes, industry news and portfolio news. For taxable accounts, complete the Advantage Cash Account Agreement and review the Advantage Terms and Conditions. For non-taxable accounts, complete the IRA Cash Agreement & Enrollment If you are opening a margin account, complete the Margin Agreement, and review the Margin Disclosure If you are transferring this account from another firm, complete and sign the Account Transfer Form and provide a copy of the last statement from the institution where your account is currently held. Depending on the type of account you are opening, additional forms may be required. Contact robin@ccsmadison.com for more information.

Trading Account (Call-in Trading)

Customers are also welcome to call-in their trades to a licensed trader at Coltrane. We have provided customers with high quality, personalized service for 20 years. And when you call us you won't hear an automated message, you will be greeted by a helpful person who can address your needs.
